I would say probably just mixing up somebody's order with somebody else and not putting the right condiment or garnishes with the order.

I would say when it has been short-staffed and we have been really busy and customer's meals are being placed in the expo line faster than I can get to them and am able to expedite customer meal tickets together. I can hear the plates crashing together as my manager is telling me to allow the other xpo workers to help me.

I apologized and they got a discount on their meal but that didn't happen that much, because I wasn't the server, although there have been times when it has been so busy that it took customer's food a little longer than normal to be expedited.
