I suggest you fill it out and submit it to them by fax (907-746-2670) with a simple cover letter.
Then be sure to call them at 907-746-1880 and followup politely. First inquire that they received the application. Then if you don't hear from them in a week or so, call back, ask if you are being considered and when you might stop in to meet someone to discuss employment options directly. You'd be amazed at how far polite follow-ups to employment apps can take you!
