Responsibilities include but are not limited to: Managing 5-7 patients daily. Assess the physical, social and psychological needs and living conditions of the patient through professional knowledge and skills of observation, interviewing and physical examination. Travel to patients’ homes and/or other care facilities (up to 75% travel). Act as the coordinator of care for the multidisciplinary team Oversees/supervises the care being provided to the patient by Licensed Practical Nurses and paraprofessionals Plus additional nursing duties

Qualifications BSN from an approved college or program License and current registration to practice as a registered professional nurse in New York required.
Minimum 1 year experience is acceptable, but 3-5 years experience is highly preferred

• Experience must come from either Hospital, Homecare, wound-care, or Long-Term Care facilities Valid driver's license
• Must be able to operate under little supervision
• Must be comfortable going into patients homes
• Must be willing to work 1st shift and 1 weekend/month Bilingual in English and Spanish is a plus, but is not required to apply. US citizenship or GC is required. Demonstrated computer skills using Microsoft and experience with an EMR system may be required based on operational needs.
