Hi Sheila, Use the space at the top sparingly - this is your first impression. Make sure it's uncluttered, that everything is accurate, and definitely make sure you include name, phone number and email address - and Jobcase profile! This has helped people get jobs, as it gives employers a well-rounded view of who you are. :)

Believe it or not, address is optional. If you are open to relocating, or have worked in several different cities, it might be wise to omit your address so that you're not pigeon-holed. As for web link, if it's work related, then yes include it.
