Required skills & experience (the "must haves" to be considered):

1. Minimum ASN degree. BLS, plus licensed Registered Nurse (RN) with no disciplinary action.
2. 1 year of Home Care experience, or private pay home care experience through an agency required
3. Must be able to participate in on-call rotation
4. Care Management Certification preferred
5. Candidate must live within Manhattan, NY
