Hey chris james , so sorry to hear about your current health issues. I think the most important thing you need to do to become more attractive to potential employers is to identify what you CAN do.
Think about your skills and identify specific jobs that you feel your current physical/health situation would make you not a good but exemplary employee in. Always keep in mind that hiring managers are responsible for hiring the very best candidates; that's their job!
Hinting any restrictions or limitations during the application and hiring process will not make you stand out as a good hire. So, make sure you take time to carefully research jobs before you apply.
