They should be supplied together if you have both. Otherwise just your resume

Depends. If it's a position with a company and the letter of recommendation is from somebody you know well that works for the company you're interviewing with, then definitely a LOR. Most people believe a LOR is only from a previous company you worked for and that's completely incorrect. But , if you don't know anybody at the company you're working for, then you must brag about yourself on your resume to build yourself up because the way a company looks at it is anybody can have their mom write a LOR about you and day they are or were your boss.
