Before you try again to look at the reviews on the play store that also says that the app is freezing or crashing even when you reinstall. Recommend that you go onto Yelp and make a comment via a local computer.

Same thing has happened to me. Send me to resume sight yet won't allow printing of completed basic form unless I subscribe and pay for different levels. Go to sights like CVS.COM.
