A lot of companies are moving to a PTO (Paid Time Off) bundle. Where they give you a bulk of time up front. That time is to be used for both sick time and vacations.
PTO upfront is beneficial to the employer because it is not accrued time and does not have to be paid out upon an employees separation from the company.
Office jobs tend to be more giving when it comes to PTO whereas blue collar jobs and jobs that rely heavily on a fully staffed operation may be less forthright with how much time they allow an employee to take.
But, to answer your question, paid time off is still a thing, though the actual amount of time varies from employer to employer.
The company I work for has unlimited paid time off. That's a nice perk. It means never having to worry about having enough days for appointments, sick time, vacations, and emergencies.
And the best part, no one is abusing the policy.

They absolutely do! It relies heavily on the position you are hired for and the company, but there are TONS of companies that provide paid location! A company that does this is aware of their employees need to balance their worklife and their personal lives.

It depends on the company. Most large corporations do. Amount of time given is based on how long you stay. For example, two weeks for first five years, then three weeks after that, then four weeks after ten years.
